Students may request official copies of their academic transcript by filing a transcript request form with the Enrollment Services Office. Request forms are located in the forms rack outside the Enrollment Services Office. Official transcripts may also be ordered by mail by sending requests to:

To avoid delays in processing, please include the following data:
- Student's Full Name (include maiden or other name if applicable)
- Current address and phone number
- Social Security Number
- Full address to which transcript is to be sent
- Student signature authorizing release of transcript
- Payment:
- $4 for one transcript
- $2 for additional transcript ordered at the same time
- $1 for each transcript over 10 copy
Checks should be made payable to CSU Stanislaus. All holds must be cleared prior to ordering transcripts (see Holds information in the schedule of classes). All transcript requests are processed on a first-come, first-served basis, with the exception of rush transcripts. You can expect 5 to 10 working days for a regular order.
Rush service available for an additional $10 fee and takes 1 - 3 working days.

Evaluation & Acceptance of Credit |

The Enrollment Services Office will evaluate previous college work as it relates to the requirement of CSU Stanislaus. Each student seeking a degree will be issued an Evaluation of Transfer Credit which will serve as a basis for determining the remaining requirements for the studentís specific objective. The evaluation remains valid as long as the student matriculates for the term specified, pursues the objective declared, and remains in additional graduation requirements unless such requirements become mandatory as a result a result of changes in the California Code of Regulations. (See "Catalog Year for Graduation Requirements" under "General Requirements for a Degree.")
Because of these regulations, a student should notify the Enrollment Services Office of any change in degree objective specified on a transfer credit evaluation.
While students may follow the specific catalog year academic requirements on which their transfer credit evaluation is based, they will be held responsible for complying with all changes in other regulations, policies and procedures which may appear in subsequent catalogs.
Candidates for Graduation
All students planning to graduate are required to file an Application for Graduation two semesters prior to the date of intended graduation. Application for Graduation forms may be obtained from the forms rack at the entrance to the Enrollment Services Office (MSR 120) (See MSR Building #27 location map) and (See Office 120 location map).
Change of Objective
Students changing their major, minor or concentration should submit a Declaration or Change of Degree Objective form available at the Enrollment Services Office (MSR 120) (See MSR Building #27 location map) and (See Office 120 location map). Assistance in choosing a major, vocational testing and counseling are available upon request from the Career Development Center at (209) 667-3381.

Release of Personal Information |

CSU Stanislaus is authorized under the Family Educational Rights and Privacy Act of 1974 to release "directory information" concerning students. "Directory information" includes name, address, telephone listing, date and place of birth, major field of study, participation in officially recognized activities and sports, weight and height of members of athletic teams, dates of attendance, degrees and awards received, and the most recent previous educational institution attended by the student. "Directory information" is also released to agencies of the State of California for recruitment purposes.
This information is subject to release at any time unless the campus has received prior written objection from the student specifying the information which the student requests not to be released. Written objections should be sent to the Director of Admissions and Records. |

Change of Address |

| Students who change mailing or permanent addresses after admissions or registration should immediately notify the Enrollment Services Office. This information is needed in order to ensure that registration information, correspondence, and other such materials reach the student without unnecessary delay. |
