How do I notify my employer of a change in address?

How do I notify my employer of a name change?

Upon receipt of your new social security (SS) card, fill out the Employee Action Request (EAR) form available on the HR website at http://www.documents.dgs.ca.gov/osp/pdf/std686.pdf You may also pick up a form in Faculty Affairs or Human Resources.  A copy of the new Social Security Card reflecting your new name must be attached to the new EAR form.  Faculty members should submit these documents to Faculty Affairs, MSR 360. Staff and management employees should submit these documents to Human Resources, MSR 320.

I’m a Staff or Management employee resigning from my employment.  What do I need to do?
Write a letter of resignation to your supervisor and copy Human Resources.  HR will contact you regarding the separation process and provide separation/clearance documents for your review and disposition.  The documents will provide information on your retirement benefits and eligibility for COBRA insurance benefits. 

How do I change my paycheck withholding exemptions?

I’ve moved to a different location on campus.  What do I do next?
Faculty members should send an e-mail to Faculty Affairs indicating your new building, room number and phone number, (if applicable).  Staff and management employees should send an e-mail to Human Resources.

How do I get my parking permit?
Annual, semester, winter term and summer permits are available for faculty, management, staff, and student parking.  Parking permit enrollment forms are available at the Cashier’s Office. Daily permits are available from various dispensers located near the entrance of each parking lot.

How do I cancel my paycheck deduction for campus parking permits?

Employees on payroll deduction for parking must go to the Cashier’s Office, MSR 100 to cancel the deduction.  The annual parking permit must be returned. The best time to cancel or go on payroll deduction for parking permit is the first of each month.

How do I obtain a Warrior Card (Faculty/Staff ID Card)?

In Turlock, faculty and staff should contact Tawn Gilihan x3419. Students should go to the Student Union Information Desk. At the Stockton campus, visit the Student Information Center. Please be sure to bring proof of your employment with the University such as a paycheck stub or your appointment letter and a picture I.D. For more details, visit the CSU, Stanislaus Warrior Card website.

How do I sign-up for my paycheck to go direct deposit?
Payroll direct deposit is available to all employees at California State University, Stanislaus except those paid on an hourly basis for appointments of 90 days of less. Once enrolled in the program, all of an employee’s payroll will be transmitted electronically to his/her financial institution.  Direct deposit enrollment forms are available from the Payroll Office or the State Controller’s website at http://www.sco.ca.gov/ppsd/empinfo.  Prior to submitting the completed form to Payroll, it is the employee's responsibility to verify the routing and account number with his or her financial institution.  Employees on any type of disability pay, on dock, or on an unofficial leave of absence, consistently closing or changing their bank account without notifying payroll, or the University is withholding a portion of a payment for any reason, will have their direct deposit cancelled administratively.

  • When is Open Enrollment?
    The open enrollment period for CalPERS health, CSU dental, Dependent Care Reimbursement Account (DCRA), Health Care Reimbursement Account (HCRA), FlexCash, and Tax Advantage Premium (TAPP) plans is usually between September 15 and October 15 each year. The effective date for all changes made during open enrollment will be January 1 of the following year. To assist employees with open enrollment choices, CalPERS mails a health plan enrollment statement to each member enrolled in a CalPERS health plan. In accordance with the health plan statement, CalPERS also has implemented a Web-based (Pacific Business Group on Health (PBGH)) “Health Plan Chooser” tool. This tool allows members to view health plans in side-by-side comparisons, and rank them based on personal references. This tool can be accessed at the following URL: http://calpers.chooser.pbgh.org/wizard.asp

  • How can I make changes to my benefit plans?
    Employees who wish to change their benefit elections during open enrollment should complete an Insurance Enrollment Worksheet and submit it to Human Resources Benefits, MSR320. The open enrollment worksheet is available by clicking this link, visiting the Benefits website, or by visiting our office. Once the worksheet has been submitted to Human Resources, enrollment documents will be prepared and you will be contacted by telephone and asked to sign the documents in Human Resources. If you have not received a call from HR within two weeks after submitting your worksheet, please call Human Resources at x3351 to confirm that your worksheet was received.
  • What are my benefits as a California State University employee?
    Benefit eligibility and the type of benefit programs you receive are based on the type of hire appointment and position.  Your initial hire letter will advise you on the type of benefits you are eligible to receive.  You should also receive a benefit packet with your appointment letter.  For specific benefit programs, please refer to the HR Benefits website at: http://www.csustan.edu/HR/Employee_Benefits/index.html and select the group your position is covered under.  For example, if you are an Administrative Support Assistant I, your position is covered by Unit 7, Clerical/Admin Support; refer to your appointment letter.

  • Will I receive insurance identification (ID) cards in the mail?
    For all health plans, ID cards will be mailed to you. Delta Dental, group #4018, and VSP (Group# 12292796), do NOT provide ID cards.

  • How can I get information about retirement?

  • When are my benefits effective?

    If you are a new hire, you have 60 days from date of hire to enroll in benefits. Your benefits will be effective the first of the month after HR receives the completed Health Enrollment Worksheet. The Benefit Enrollment Worksheet is available on the Employee Benefits page of the HR website at http://www.csustan.edu/HR/Employee_Benefits/index.html

  • What do I need to do to add or delete a dependent?
    Complete the Insurance Enrollment Worksheet available on the Employee Benefits page of the HR website at http://www.csustan.edu/HR/Employee_Benefits/index.html.  If you are a staff or management employee, submit the form to Human Resources.  If you are a faculty employee, submit the form to Faculty Affairs.

  • I would like to utilize the fee waiver benefit.  What do I do?
    First, you'll want to verify that you are eligible. If eligible, complete an Employee Fee Waiver Application or Dependent Fee Waiver Application. If you are a staff or management employee, submit the form to Human Resources.  If you are a faculty member, submit the form to Faculty Affairs.  If you are using an employee fee waiver for the first time, also submit an approved Individual Career Development Plan to Human Resources. Information about the Fee Waiver Program, eligibility criteria, and all related forms are available on the Human Resources website at: http://www.csustan.edu/hr/Employee_Benefits/General/FeeWaiver.html.  Please call 209/667-3351 with any questions.

  • My medical/dental/vision insurance plan does not show coverage for me and/or my dependent(s).  What should I do?

    There can be a delay between your actual date of coverage and the day your medical/dental/vision plan receives your enrollment data.  In most cases, this should be no more than two weeks from the date your plan(s) appear on your paycheck stub.  If you need emergency/immediate services, you may choose to pay cash for your services and request reimbursement from your insurance plan once they have received your enrollment data. 
    If you need assistance please contact Human Resources at 667-3351 (staff & management employees) or Faculty Affairs at 667-3028 (faculty employees) to confirm your date of coverage and for assistance. Group plan information and contact phone numbers are available on the Employee Benefits page of the HR website at: http://www.csustan.edu/HR/Employee_Benefits/index.html

  • How do I start/stop/change a TSA 403(b)? 457 or 401(k)? Savings Bond?
    TSA 403(b): To start contributing you must contact one of the authorized companies and have an account established.  You can find this list of companies at: http://www.csustan.edu/HR/Employee_Benefits/index.html. Once your account has been established, you must complete a Salary Reduction Agreement (also available at the above link) and submit it to Human Resources (staff & management employees) or Faculty Affairs (faculty employees).  To make changes to or cancel an existing TSA, you simply complete a Salary Reduction Agreement indicating the change and submit to Human Resources or Faculty Affairs.

  • 457 or 401(k): To start contributing, you must enroll through the Savings Plus Program.  You can find the enrollment form at http://www.csustan.edu/HR/Employee_Benefits/index.html.  Once the Savings Plus Program has established your account, you will receive a packet of information in the mail listing all of your investment options.  You will also receive instructions on how to activate your account either online or by telephone.  Once you have activated your account, you can make changes to it either online or by telephone.

  • Savings Bond: To start contributions, you must complete the authorization form.  You can find this form along with instructions at: http://www.documents.dgs.ca.gov/osp/pdf/std242.pdf.  Once you have completed the form you will submit it to Human Resources (staff & management employees) or Faculty Affairs (faculty employees).  To make changes you simply follow the same steps.

  • Do you have discount tickets for the various amusement parks and activities?

  • How do I change/cancel my voluntary miscellaneous payroll deductions?
    Contact the company/organization directly.  The Insurance Company Contact Information page lists the phone numbers and websites. The company/organization will submit a cancellation notice directly to the State Controller’s Office who administers CSU payroll. Employees must also submit a written cancellation request to the State Controller’s Office.   • Maternity/Paternity Leave

  • What is the maximum amount of leave I can take?
    A permanent female employee may take up to one year of leave for pregnancy, childbirth or recovery.  This leave may be paid or unpaid depending on benefits available and is not predicated upon disability. (Ed Code 89519)

  • What is the maximum amount of PAID leave I can take?
    This will depend on several things such as how much sick leave you have accrued; whether you choose to apply for Non-Industrial Disability Insurance (NDI); how many days of paid maternity leave are available to you; and how much vacation you have accrued.

  • Is my job protected while I am on leave?

    There are federal and state laws that protect a woman’s right to take maternity and family leave.  There are also provisions in the CSU Collective Bargaining Agreement and CSU Policies regarding maternity and family leave.  Since the application of these laws, provisions & policies can be complex, it is best to make an appointment with Kelly Mode, Human Resources, MSR320, kmode@csustan.edu, to discuss your specific situation.

  • Do I need to request my leave in writing?
    Yes, you should put your anticipated leave dates in writing to your supervisor and cc Human Resources.  You should include your anticipated last day of work and return to work date.  Of course, if your doctor should take you off work early for medical reasons, these dates may need to be adjusted.

  • Do I need a note from my doctor?
    When your doctor takes you off work, you must provide Human Resources a written statement from him/her excusing you from work.

  • Can I change my return to work date once I’ve started my leave?

    In most cases you must indicate your return to work date when you begin your leave.  Generally your leave may be extended beyond your original return to work date for medical necessity only.

  • How soon do I need to enroll my new child in health benefits?
    A newborn is covered under his/her mother’s health plan for the first 30 days.  You must provide your child’s name & birthday to Human Resource (staff & management employees) or Faculty Affairs (faculty employees) within 30 days of birth in order to maintain continuous coverage.

  • How will I get my paycheck while I am on leave?
    You may make arrangements with your department or contact payroll to have your paycheck mailed to your home while you are on leave.

How do I get a job application for a staff job opening at the University?
All campus position announcements, applications and application procedures are available on the HR Website (www.csustan.edu/hr).  In addition, our job line 209/667-3354 lists the current openings and procedure for obtaining an application.  Job announcements and applications are also posted in Human Resources (MSR 320) and at the CSU Stanislaus - Stockton campus in the student lounge, Acacia Building .

How long is my temporary pool application considered active?

Temporary Pool applications are considered active for 6 months or until you fill an emergency hire appointment.  It is the applicant’s responsibility to keep their application current and resubmit a new application after such time has elapsed or after you have completed an assignment.

Do I have to fill out a separate application for each job I wish to apply for?
Yes.  It is the applicant’s responsibility to apply for each job they are interested in. Please be sure to indicate the job number on each application.

How can I check on the status of the job I applied for?
You will be notified of the status of your application by completing the Applicant Notification Form as part of your application packet.  Applicants interviewed and not selected for a position will receive a letter of notification to that effect.  You may call 209/667-3351 with the job number and position title to ask about the status of a particular recruitment.  Please note that screening of applications by the search committee typically takes three weeks following the application deadline.

When is the next workshop going to be held?
Please refer to the Training and Development page on the HR website. From this page you can link to the Professional Development Training Calendar where scheduled classes are listed.  As other classes are confirmed, the calendar is updated on a continuous basis.

How do I register, and how is my workshop registration confirmed?
The Professional Development Training Calendar provides information on how to register. For most classes, you will complete the Training Registration Form, have your supervisor sign it, and forward to Human Resources. Once your approved registration is received in Human Resources, you will be notified by email.

Is there a cost to attend a workshop?
Most workshops are free of cost.  However, those that have a registration fee, e.g. desktop application classes, will indicate on the workshop summary any costs associated with the class.  Travel expenses are in addition to the registration fee and are paid by your department if the training and related travel are approved.  The Professional Development Training Calendar on the Human Resources website contains all relevant information for scheduled workshops.  Please call 209/667-3351 with questions regarding specific workshops.
